DOCUMENT TITLE: RELEASE & WAIVER OF LIABILIT & INDEMNITY AGREEMENT

In consideration for being permitted to participate in any way in the Martial Arts Program indicated below and/or being permitted to enter for any purpose any area within the facility operated by IPPON Martial Arts (IMA), the applicant whose signature appears below agrees as follows:

1. I fully understand and acknowledge that:
   1.  There are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury, partial and/or total disability, paralysis, or death.
   2.  The social and economic losses and/or damages that could result from the risks and dangers of martial arts training could be severe.
   3. The risks and dangers of martial arts training may be caused by the action, inaction, negligence, or otherwise of the applicant, other students, non-participants, or agents, officials, employees or representatives of IMA.
   4. There may be other risks and dangers not known or reasonably foreseeable to the applicant, other students, non-participants, or agents, officials, employees or representatives of IMA at this time.
2. I accept and assume responsibility for all such risks and dangers and for any losses or damages of any nature that may result from my participation in the training, practice, activities, or events conducted by IMA.
3. I hereby release, waive, discharge, and covenant not to sue IPPON Martial Arts, its owners, managers, promoters, agents, employees, or representatives, lessees or lessors of the IMA premises, for any losses or damages of any nature that may result from my participation in the training, practice, activities, or events conducted by IMA, on behalf of myself, my personal representatives, assigns, executors, heirs, or next of kin.
4. I acknowledge that any injuries or damages that I may receive in the course of my participation in the training, practice, activities, or events conducted by IMA may be compounded or increased by rescue or assistance measures. I hereby release, waive, discharge, and covenant not to sue the JMAC, its owners, managers, promoters, agents, employees, or representatives, lessees or lessors of the IMA premises, for any losses or damages of any nature that may result from any attempts to assist or rescue me after I have sustained injury or damage during the course of my participation in the training, practice, activities, or events conducted by IMA.
5. This Release and Waiver of Liability and Indemnity Agreement is intended to be a broad and inclusive as allowed by law. Should any portion be found to be unenforceable, the remainder shall continue in full force and effect, and the intent of the document shall be enforced to the maximum extent allowed by law.
6. The undersigned understands and agrees that because class space is very limited at IPPON Martial Arts (IMA), no refunds can be given.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I WAIVE SIGNIFICANT RIGHTS BY SIGNING IT. I HAVE SIGNED IT FREELY AND VOLUNTARILY IN CONSIDERATION FOR THE PRIVILEGE OF BEING ADMITTED TO THE TRAINING PROGRAMS AT IPPON MARTIAL ARTS AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF
ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
